This template is not for blatant violations of the username policy, it is only for edge cases or questionable usernames. Report blatant violations to WP:UAA.

Usage:

{{subst:Uw-username|Reason}} references a specific reason

{{subst:Uw-username|Reason|Additional text}} adds text onto the end of the message instead of "Thank you"
